How to File an FBN Statement in Santa Clara County

1. Search the name. Use the county's official FBN name search to check whether the name is already on file.
2. Complete the statement. Follow the Santa Clara County Clerk-Recorder's Office's FBN instructions, listing every business name and every registrant. Complete the form in black ink only. The county allows up to 5 workdays to process an application, and a statement sent without payment is returned.
3. File and pay the county fee. The county charges $40 (the clerk's fee, which can change — check the county page before filing) for one business name and one registrant. That $40 covers one business name and one or two owners; each additional business name or owner on the same statement is $7.
   • In person: Clerk-Recorder's Office, Business Division, 110 West Tasman Drive, First Floor, San Jose, CA 95134 (walk-ins accepted; the county suggests booking an appointment)
   • By mail: the same address, with a self-addressed stamped envelope and a check payable to the Clerk-Recorder's Office
   • By email: the county accepts completed applications at bus-rdc@rec.sccgov.org
4. Keep your stamped copy. The clerk stamps it with a file number and filing date. That date starts your 45-day publication clock.
